Diagnosis and Treatment of Cervical Radiculopathy

Cervical radiculopathy is a neurological condition that occurs when a nerve root in the cervical spine becomes compressed or irritated. It can cause neck pain radiating into the shoulder and arm, numbness, tingling, or muscle weakness in the upper limb.

Common causes include a cervical herniated disc, degenerative changes of the cervical spine, and narrowing of the spaces through which the cervical nerve roots exit.

Symptoms depend on which nerve root is affected. For this reason, an appropriate neurological assessment is important both for establishing the diagnosis and selecting the most suitable treatment.

What Is Cervical Radiculopathy

Nerve roots emerge from the spinal cord through the cervical spine and contribute to the motor and sensory function of the shoulders, arms, hands, and fingers.

When one of these nerve roots becomes compressed or irritated, symptoms may develop along the area supplied by that nerve.

Pain may begin in the neck and radiate towards the shoulder blade, shoulder, upper arm, forearm, or fingers. In some patients, the main symptom may not be pain but rather numbness, tingling, or weakness in the arm or hand.

Symptoms of Cervical Radiculopathy

The symptoms of cervical radiculopathy vary depending on the nerve root affected and the severity of the compression or irritation.

Symptoms may include:

  • neck pain
  • pain radiating towards the shoulder blade or shoulder
  • pain extending into the upper arm, forearm, or fingers
  • numbness or tingling in the arm, hand, or fingers
  • burning or electric shock-like sensations along the upper limb
  • muscle weakness in the arm or hand
  • reduced grip strength
  • changes in upper-limb reflexes
  • worsening of symptoms with certain neck movements or positions

The distribution of pain, numbness, and muscle weakness can provide important information about which cervical nerve root may be affected.

 Cervical Disc Herniation and Cervical Radiculopathy

A cervical herniated disc is one of the possible causes of cervical radiculopathy.

The intervertebral discs are located between the cervical vertebrae and function as flexible cushions, helping to absorb mechanical stress and allowing movement of the spine.

When part of a cervical intervertebral disc extends beyond its normal boundaries, it may irritate or compress a nearby nerve root.

However, the presence of a herniated disc on an MRI does not necessarily mean that it is responsible for the patient's symptoms. Imaging abnormalities can also be present in people without pain. For this reason, MRI findings should always be interpreted together with the patient's symptoms and neurological examination.

Causes of Cervical Radiculopathy

Cervical radiculopathy may result from different conditions that reduce the available space around a nerve root or cause irritation of the nerve.

Common causes include:

  • cervical disc herniation
  • degenerative changes of the intervertebral discs
  • cervical spondylosis
  • formation of bone spurs (osteophytes)
  • narrowing of the intervertebral foramina
  • less commonly, trauma or other disorders affecting the cervical spine

In younger adults, cervical radiculopathy may be more commonly associated with disc herniation, while in older adults it is often related to degenerative changes of the cervical spine.

Cervical Radiculopathy or Another Neurological Condition

Pain, numbness, and weakness in the arm or hand are not always caused by a problem in the cervical spine.

Similar symptoms may occur in conditions such as:

  • carpal tunnel syndrome
  • ulnar nerve entrapment
  • other peripheral neuropathies
  • brachial plexus disorders
  • shoulder disorders
  • cervical myelopathy
  • other neurological or musculoskeletal conditions

Differential diagnosis is particularly important because treatment varies considerably depending on the underlying cause of the symptoms.

Diagnosis of Cervical Radiculopathy

The diagnosis of cervical radiculopathy is initially based on the patient's medical history and a detailed clinical and neurological examination.

Particular attention is given to the pattern of pain, distribution of numbness, presence of muscle weakness, and changes in reflexes.

Diagnostic assessment may include:

Neurological examination: Muscle strength, sensation, reflexes, coordination, and other neurological functions are assessed to help identify the affected nerve root.

Cervical Spine Magnetic Resonance Imaging (MRI): MRI can provide detailed images of the intervertebral discs, spinal cord, nerve roots, and surrounding structures. It may identify a herniated disc, spinal narrowing, or another potential cause of nerve compression.

Electromyography and Nerve Conduction Studies (EMG/NCS): In selected cases, these tests can help confirm radiculopathy and, importantly, differentiate it from peripheral nerve disorders such as carpal tunnel syndrome or ulnar neuropathy.

Not every patient with neck or arm pain requires immediate MRI or electromyography. The need for further investigations is determined by the symptoms and findings of the neurological examination.

Treatment of Cervical Radiculopathy

The treatment of cervical radiculopathy is individualized according to the underlying cause, duration and severity of symptoms, and the presence of muscle weakness or other neurological deficits.

In many cases, particularly when there is no significant or progressive neurological deficit, initial treatment is conservative.

Treatment may include:

  • appropriate medication for pain relief
  • treatment for neuropathic pain when indicated
  • physiotherapy
  • individualized mobility and strengthening exercises
  • ergonomic adjustments at work and during everyday activities
  • modification of activities that aggravate symptoms
  • gradual return to normal physical activity

Prolonged immobilization of the neck is generally not considered an appropriate long-term solution. Suitable movement and rehabilitation, according to the patient's individual condition, are important components of treatment.

When radicular pain persists despite appropriate conservative treatment, additional specialized therapeutic options may be considered.

Indications for Surgical Treatment

Most cases of cervical radiculopathy do not require surgery.

However, surgical evaluation may be necessary when there is:

  • significant or progressive muscle weakness
  • severe or worsening neurological deficit
  • persistent and significant radicular pain despite appropriate conservative treatment
  • significant nerve root compression corresponding to the clinical symptoms
  • spinal cord compression or signs of cervical myelopathy

The decision regarding surgical treatment is based on the combination of symptoms, neurological findings, duration of the condition, and imaging results.

When to Seek Urgent Neurological Evaluation

Certain symptoms may indicate more significant involvement of the nerve structures or spinal cord and require prompt medical assessment.

Particular attention is required when there is:

  • new or rapidly worsening weakness in one or both arms
  • weakness or sensory disturbances affecting the lower limbs
  • difficulty walking or problems with balance
  • loss of hand dexterity or difficulty performing fine movements
  • significant problems with coordination
  • new bladder or bowel dysfunction
  • severe pain following significant trauma
  • neck pain accompanied by fever or other significant systemic symptoms

Symptoms affecting both the upper and lower limbs, walking difficulties, or loss of hand dexterity may raise suspicion of cervical myelopathy, which requires timely neurological evaluation.

Neurological Follow-Up of Cervical Radiculopathy

Neurological follow-up is important when cervical radiculopathy is associated with persistent pain, numbness, or muscle weakness.

During follow-up, the progression of symptoms, muscle strength, sensation, and reflexes can be assessed, together with the patient's response to treatment.

Accurate diagnosis of cervical radiculopathy and cervical disc herniation, combined with individualized treatment and appropriate rehabilitation, can help reduce pain, improve neurological function, and support the patient's return to normal daily activities.
