The Wender Utah Rating Scale (WURS) is used to diagnose hyperactivity disorder and attention deficit disorder (ADHD) in children aged 5 to 17 years.
The Wender Utah Rating Scale (WURS) consists of 18 questions about the child's behavior over the last 6 months.
The questionnaire is intended to be completed by parents and teachers of the child or adolescent. The first elements raise questions about behavior related to carelessness (eg the child has difficulty organizing tasks and activities). The second set of questions raises questions about the behavior associated with symptoms of hyperactivity and impulsivity (eg speaking too much). The last set of questions raises questions as to whether the behavior symptoms were present before the age of seven. It is also useful in detecting the sub-type (inattention or hyperactivity) of the disorder.
