Fibromyalgia is a chronic condition characterized primarily by widespread musculoskeletal pain, increased sensitivity, fatigue, sleep disturbances, and often difficulties with concentration and memory. These symptoms can significantly affect daily functioning, work, and quality of life. Fibromyalgia can affect both men and women and can occur at different ages; however, it is diagnosed more frequently in women. Fibromyalgia is not simply a disorder of the muscles or joints. Current understanding suggests that changes in the way the central nervous system processes and regulates pain signals play an important role. For this reason, fibromyalgia is closely associated with altered or increased pain processing.
Symptoms of Fibromyalgia
The clinical presentation of fibromyalgia can vary considerably from one patient to another. Symptoms often fluctuate, with periods of improvement and exacerbation. The most characteristic symptom is chronic widespread pain, which may affect multiple areas of the body.
Common symptoms include:
- widespread musculoskeletal pain and tenderness
- stiffness, particularly after periods of inactivity
- chronic fatigue
- unrefreshing sleep
- difficulty with concentration and memory
- a feeling of “mental fog”
- headaches or migraine in some patients
- numbness or tingling without necessarily having peripheral neuropathy
- increased sensitivity to various sensory stimuli
- anxiety or depressive symptoms
- significant reduction in stamina and daily functioning
Other chronic symptoms or syndromes may also coexist, including irritable bowel syndrome, chronic pelvic pain, and sleep disorders.
Chronic Pain and Fibromyalgia
Fibromyalgia pain is often described as deep, widespread, or burning and may vary in both intensity and location. Unlike pain caused by a specific injury or localized inflammation, fibromyalgia pain may affect multiple areas of the body without a corresponding structural abnormality that fully explains its severity. This does not mean that the pain is not real. On the contrary, the current understanding of fibromyalgia recognizes important alterations in the mechanisms responsible for pain processing and regulation. Increased sensitivity of the nervous system to painful stimuli, and sometimes even to normally non-painful sensory stimuli, is an important feature of the condition.
Fatigue and Sleep Disturbances
Fatigue is one of the most common and disabling symptoms of fibromyalgia. Even after several hours of sleep, a patient may wake up without feeling rested. Unrefreshing sleep can worsen pain, fatigue, and cognitive difficulties. Some patients may also have separate sleep disorders, such as insomnia, restless legs syndrome, or obstructive sleep apnea. Identifying and appropriately treating a coexisting sleep disorder can therefore be an important part of fibromyalgia management.
Cognitive Difficulties
Many patients with fibromyalgia report difficulties with concentration, attention, short-term memory, or the speed at which they process information. These symptoms are commonly referred to as “fibro fog.” Cognitive difficulties may become more noticeable when pain, fatigue, poor sleep, anxiety, or depression are more severe. When memory problems or other cognitive difficulties are significant or have unusual characteristics, appropriate evaluation may be necessary to exclude other possible causes.
Causes and Factors Associated with Fibromyalgia
The exact cause of fibromyalgia is not fully understood, and there is probably no single mechanism that explains all cases. Current research suggests that alterations in the brain and spinal cord systems involved in the perception and regulation of pain play an important role.
In some people, the onset or worsening of symptoms may be temporally associated with factors such as:
- physical injury
- infection or another illness
- prolonged physical or psychological stress
- sleep disturbances
- significant stressful events
In other patients, symptoms develop gradually without a clearly identifiable triggering event.
Diagnosis of Fibromyalgia
The diagnosis of fibromyalgia is primarily clinical and is based on the patient's medical history, the characteristics and duration of the symptoms, and an appropriate clinical examination. There is no single blood test or imaging examination that can independently confirm a diagnosis of fibromyalgia. The assessment focuses on the presence of chronic widespread pain and associated symptoms such as fatigue, sleep disturbances, and cognitive difficulties.
Investigations and Differential Diagnosis
Depending on the symptoms and findings of the clinical examination, additional investigations may be required to identify other conditions that can cause similar symptoms.
The differential diagnosis may include, among others:
- hypothyroidism
- inflammatory rheumatic diseases
- rheumatoid arthritis
- systemic lupus erythematosus
- polymyalgia rheumatica in appropriate age groups
- myopathies
- anemia or specific nutritional deficiencies
- neurological disorders
- sleep disorders
- other conditions that cause chronic pain or fatigue
Laboratory tests and imaging studies are selected according to the clinical presentation, and not every patient with fibromyalgia requires extensive diagnostic testing.
Fibromyalgia, Anxiety and Depression
Anxiety and depression are more common among people living with chronic pain and may coexist with fibromyalgia. However, fibromyalgia should not be considered simply a psychological or psychiatric disorder. Mental health assessment can be useful when relevant symptoms are present, as chronic pain, sleep, mood, and anxiety interact with one another and can significantly affect quality of life. When clinically indicated, appropriate psychometric tools may be used to assess anxiety, depression, and other psychological factors. However, extensive personality testing is not required for the diagnosis of fibromyalgia.
Treatment of Fibromyalgia
The treatment of fibromyalgia is usually multifaceted and individualized according to each patient's predominant symptoms and individual needs. There is no single treatment that is effective for everyone. The best results are often achieved through a combination of non-pharmacological approaches and, when necessary, medication.
Important elements of treatment may include:
- education and psychoeducation about the condition
- gradual and regular physical activity
- aerobic exercise and muscle strengthening
- improving sleep quality
- stress management
- maintaining a healthy lifestyle
- Cognitive Behavioral Therapy (CBT) in appropriate patients
- treatment of coexisting problems such as anxiety or depression
Physical activity usually needs to be increased gradually, as excessive exertion at the beginning may temporarily worsen symptoms.
Pharmacological Treatment of Fibromyalgia
Medication may be considered when symptoms remain significant despite appropriate non-pharmacological interventions. Depending on the predominant symptoms, certain medications that influence chronic pain mechanisms may be used and can sometimes also help with sleep or other associated symptoms.
The choice of medication is individualized according to:
- the intensity and characteristics of the pain
- sleep disturbances
- the presence of anxiety or depression
- the patient's age
- coexisting medical conditions
- other medications being taken
- potential side effects
Simple analgesics are not always particularly effective for fibromyalgia, while opioids are generally not considered a first-line treatment for its long-term management.
Exercise and Rehabilitation in Fibromyalgia
Appropriately adapted exercise is an important component of the long-term management of fibromyalgia. Depending on the patient's abilities and preferences, this may include aerobic activity, walking, swimming, strengthening exercises, or other forms of gentle exercise that are gradually increased over time. The aim is not intense physical exertion, but rather the progressive improvement of physical fitness, endurance, and functional ability. An individualized approach is particularly important for patients who have significantly reduced their activity because of chronic pain or fatigue.
Follow-Up of Fibromyalgia
Follow-up of fibromyalgia should not focus exclusively on the intensity of pain.
It is also important to assess:
- sleep quality
- fatigue
- physical activity
- mood
- cognitive function
- occupational and social functioning
- response and tolerance to treatment
The development of new objective muscle weakness, significant sensory disturbances, walking difficulties, fever, unexplained weight loss, or other new neurological or systemic symptoms requires further investigation and should not automatically be attributed to fibromyalgia. Accurate diagnosis and individualized treatment of fibromyalgia can help reduce chronic pain and fatigue, improve sleep and physical functioning, and support better management of everyday life.
